Insurance and Payments

image At Oquirrh Mountain Eye Care our goal is to promote great eye health and assist you in having the clearest vision possible. We are currently accepting several types of medical insurance plans as well as vision plans, check, cash FSA, HSA, care credit and most major credit cards. You do not have to have insurance or a vision plan to be seen in our office, we do accept self-pay patients. Our front desk staff can assist you with a breakout of appointment and material costs at the time of your appointment.

Vision Insurance

The knowledgeable staff at Oquirrh Mountain Eye Care is experienced with working with vision plans and able to assist you during your visit to our office with questions you may have about coverage. We will work closely with you and your vision plan provider to submit claims regarding your vision exam and materials purchase. We strive to ensure that you are taken care of and can help with any questions you may have with your individual vision plans. Many vision plans are designed to cover the basic vision related costs, however not all costs related to eyecare are covered by vision plans. Most vision plans do require a co-payment for the office visit as well as a portion of the total material cost and sales tax. Medical Insurance Policies Currently Accepted at Oquirrh Mountain Eye Care (you will want to check with your individual medical insurance provider and verify that they cover a preventative eye exam on your policy, we are unable to verify that information in our office):

  • Select Health Medicaid (we cannot accept new patients at this time with this coverage, you must be an existing patient to be seen)

Vision Insurances

  • VSP (Vision Services Plan)
  • EyeMed
  • Davis Vision (we are currently not accepting new patients with this coverage; you must be an existing patient to be seen)

FSA - Flexible Spending Accounts & HSA - Health Savings Accounts

Many employers are offering FSA or HSA as a benefit to their employees. Sometimes these are referred to as cafeteria plans and are elective, supplemental insurance plans. These plans are designed to let you set money aside for medical expenses pre-taxed from your annual pay. These funds can be used to pay for medical expenses such as co-pays, eye exams, glasses, contacts, and eye surgeries. Check with your benefits administrator at your workplace to see if you are eligible for these programs. Both of these programs are accepted at Oquirrh Mountain Eye Care as a form of payment.

Oquirrh Mountain Eye Care requires payment at the time of service. Our office accepts the following forms of payments:
